SSRI-induced apathy syndrome

"Loss of the ability to feel excitement after Lexapro? Anhedonia"

"All SSRI's can have an impact on dopamine and do so at a reasonably high incidence. There are two known (there are likely more, not very well understood) mechanisms by which this can occur;

Increase in serotonin availability activates 5-HT2C which inhibits DA activity

Dopamine transport protein has weak affinity for serotonin, increase in serotonin availability increases the chance of the protein accidentally binding to serotonin blocking dopamine transport

The condition you are experiencing is called SSRI apathy syndrome. It can persist even when you stop taking the drug.

In terms of next steps to discuss with your doctor;

You should get a genetic test to check if you are predisposed to unusual CYP's function. These are the enzymes that metabolize most drugs and between 1% and 5% of people have a problem with CYP450 which handles SSRI's. This manifests as unusual serum levels (either too high or too low) which either makes the drugs ineffective or you end up over medicated. This would also explain why Wellbutrin isn't helping.

You may need to do a microtaper. If you are in the US you would use a compounding pharmacy to do this.

There are newer drugs that don't have the same spillovers as SSRI's like Trintellix"
