Invitation to survey on personal experience with PSSD

Hello,
I am reaching out from The University of Iowa Hospital and Clinics' new PSSD research team. We would like to learn more about the symptoms, precipitating medications, and potential medical therapies for PSSD. Our goals include better characterizing the impact that PSSD has on the individual and bringing awareness of this condition to the medical community. We would like to invite individuals with PSSD to complete the short IRB-approved survey. Permission to share this survey was obtained prior to posting.

That's usually an institutional review board (IRB) requirement. Especially in the USA.
It is also a bioethics requirement, then you are informed of the scope of the study and how the data will be used.
Volunteers must provide their written consent when participating in any research which involves their medical condition.

The signature is required by the Internal Review Board (IRB) as someone mentioned below. It is not shared with anyone. It remains in the database which is behind the University of Iowa's firewall and has strong restrictions on who is allowed to access any part of the data collected from studies. The signature is so that we know you have read the consent form as it is important that everyone knows the research is voluntary. I completely understand if you wish to not provide a signature. Privacy is important. If you ever have questions or concerns please feel free to reach out.

I had completed it too. Not sure why more people did not jump on it. Of course you need a signature and who cares?
